Changes in Personal Information

Follow these steps to change the personal information of your policy.

Step 1 : Choose the correct Service Request Form and complete all the fields accurately
Step 2 : Provide all required official signatures
Step 3 : Provide the necessary identity documents
Step 4 : Submit the completed form(s) together with supporting documents via your Financial Consultant or email to Customer Service Centre together with scanned supporting documents.
Alternatively, you may mail it to us, attention to “Customer Service & Operation Department”.

 

If you want to make correction in name, date of birth, email id, phone number, address etc., require the following documents:

  • A completed and signed Change of personal details Form

  • Complete name change needs to be attached with additional proof of documents.

  • A copy of NRC Card/ passport as ID proof (mandatory)

  • Address proof like NRC/ Passport/ household statement/others

  • A copy of Birth Certificate or passport copy for Date of Birth change

  • A copy of Household statement (Optional)

  • Recommendation letter from the local authorities that validates the information provided (Optional)

Changes in Nominations

Follow these steps to change the nominations of your policy.

  1. Choose the correct Service Request Form and complete all the fields accurately 

  2. Provide all required official signatures

  3. Provide the necessary identity documents

  4. Submit the completed form(s) together with supporting documents via your financial consultant or email to Customer Service Centre together with scanned supporting documents. Alternatively, you may mail it to us, attention to “Customer Service & Operation Department”. 

When you, the policyowner, make a revocable nomination, you will continue to retain full rights and ownership over the policy. This means that you can change or revoke a nomination at any time without the consent of the beneficiary or beneficiaries. All rights over the policy will be subjected to the terms and condition of the nomination created.

 

To nominate immediate family members who have insurable interest like spouse, parents, children, grandparents, the following documents are required.

  • A completed and signed Change of Nomination/Beneficiary Form

  • A copy of NRC Card/ Passport as ID proof (mandatory)

  • A copy of Birth Certificate (Optional)

  • A copy of Household statement (Optional)

If the designated beneficiary is younger than 18 years old, the policyholder/ proposer is required to nominate the guardian like Life Assured’s parents, grandparents, or immediate family member as an appointee or trustee on the request form. 

To nominate other relatives or entities other than immediate family members, the case would need to be reviewed.

  • A completed and signed Change of Nomination/ Beneficiary Form

  • A copy of Household statement (Optional)

  • A written declaration explaining the reason for nominating the stated beneficiary(ies) who does not have any insurable interest.

Changes in Specimen Signature

Follow these steps to change the specimen signature of your policy.

  1. Choose the correct Service Request Form and complete all the fields accurately 

  2. Provide all required official signatures

  3. Provide the necessary identity documents

  4. Submit the completed form(s) together with supporting documents via your financial consultant or email to Customer Service Centre together with scanned supporting documents. Alternatively, you may mail it to us, attention to “Customer Service & Operation Department”. 

 

If you want to change Specimen Signature, the following documents are required:

  • A completed and signed Specimen Signature Registration Request Form

  • Both old and new signatures need to be attested.

Life assured’s overseas travel of over 3 months (to hide at the moment)

If the Life Assured travels overseas (for over 3 months), the payment will be made by your representative. 

If the policyowner or proposer is the same person as the Life Assured, the following documents are required:  

  • A copy of passport and visa of the Policyowner/Life Assured

  • The representative should be his close family member 

  • A copy of NRC card of the premium payment representative

  • A household statement copy along with a written declaration, by the Policyowner/Life Assured, explaining the relationship between the Policyowner/Life Assured and the premium payment representative. 

 If the Policyowner is not the same person as the Life Assured (and, Policyowner is within the country), the following documents are required:  

  • A copy of passport and visa of the life assured 

  • No other supporting documents or nomination of premium payment representative is required.

Changes in Policy Assignment

If you want to assign the policy to any institution or individual for the consideration, require the following documents:

  • A duly completed and signed <Assignment form>

  • A copy of NRC Card/ Passport as ID proof

  • Notice of assignment and deed of assignment

If you want to do Conditional Assignment 

  • A duly completed and <Assignment form>

  • A copy of NRC Card/ passport as ID proof (mandatory)

  • A copy of Household statement (Optional)

  • A written declaration explaining the reason of nominating the stated beneficiary(ies) who is not having any insurable interest.

Change in Policy Details

You may request policy details via the following options:

Request to Surrender the Policy

Policy holder/ proposer can request to surrender their policies at any moment before the stated maturity dates through Financial Consultant or our Customer Service Center. Once a policy has been surrendered, the policy is terminated, and thus no further insurance coverage is provided.

Important notes before submission
  1. Ensure all fields are completed and your signature is executed in the same manner as in the Proposal Form or your latest Signature Declaration Form.

  2. Attach relevant supporting documents (if any), e.g. proof of identity. You may contact your financial consultant on the relevant supporting documents needed for your application.

How do I submit the service request form?

What happens after I submit my service request?

 Once we receive your service request, we will process it based on the terms & conditions of the policy. When a decision has been made or in the event that we require further clarification of details, we will notify you & your Financial Consultant in writing.

How do I check the progress of my application?

You may check the status of your application via the following options:

  • Contact our Customer Service Hotline

What is a policy anniversary?

The Policy Anniversary refers to the month when the proposal commences to become a policy. For example, the term of the plan contract starts on 01st January 2020, the plan will expire on 31st December 2020 23:59:59.

What happens if I do not pay my policy premium on time?

Your policy might lapse. You will not have insurance protection/ coverage, and will also not be entitled for any insurance claim during the lapse period.

Can I nominate more than one person?

Yes, you can. You need to state the specific share (%) to be paid to each of the nominees. Unless stated, the policy monies will be divided equally among all nominees.
